Each participating practice will identify a registry manager to serve as the primary point of contact for administrative communications regarding the registry. The registry manager will have the authorization to designate data entry privileges for personnel to enter and view patient data in the registry and receive registry reports for practice improvement activities.
Technology Requirements
The only technology requirements for participation are a computer with a web browser, Adobe Acrobat version 6.0 or higher, an e-mail account, and an internet connection.
