Participation Agreement
This agreement establishes the terms of participation, including the HIPAA required business associate agreement, and lists each clinician who will participate in the registry. Participant agreements are for one year.
Fees
Annual Participation Fees (Per Clinician)
Participation fees are assessed according to the number of clinicians within a practice who will participate in the registry. The annual fee schedule for participating clinicians is as follows:
- AGA Members: $200/individual provider/per year
- Non-Members: $350/individual provider/per year
PQRS Optional Add-on
Hepatitis C measures group: $150/individual provider/per year
